Thank you for the honor of serving you as your Secretary-Treasurer for 2007-2009. I appreciate your support and it is my pleasure to be a part of the Central Region Team.I am here to serve you. Please feel free to contact me if you should have questions or concerns.

Each chapter is required to complete its financial reports for 2007-2008 by October 31, 2007.Please submit separate checks to National and Regional officers. Do not combine the check amounts. The following forms must be completed:

- Teen Roster (can be printed from MIS) 

- Life Membership Form (for new life members only)

- National Operating Account Bank Transmittal 

Please note that copies of these forms should be disbursed as follows:

1. Send one (1) copy of all completed forms and two (2) copies of the National Operating account Bank Transmittal form along with your chapter check to:

2. Send one (1) copy of all forms along with your chapter check to :

3. Retain a copy of all reports for your records.

4. Send a copy of your National Operating Account Bank Transmittal form along with a copy of your chapter’s check to:

CHECKLIST FOR REPORTING

1. Per Capita Tax and Bonding - Due by October 31, 2007

Payable to: Jack and Jill of America, Inc. - National

_ National Per Capita Tax 

_ Members and Associates $75.00

_ National Life Membership $250(10yrs), $200(15yrs) or $150(20yrs) 

_ Bonding Fee $10

Payable to: Jack and Jill of America – Central Region 

_ Regional Per Capita Tax, Members and Associates Regional - $55

_ Teens Regional -$25

_ Regional Life Membership $200, $150, or $100

_ Chapter Courtesy $50

2. Print the following lists from our MIS program and forward to National Treasurer at Headquarters and to me, Central Region Secretary/Treasurer.

_ Active Membership Roster 

_ Associate Membership Roster

Check the appropriate box for an Associate if Paid Life Member or New Life Member paying for the first time with this reporting period. (If a New Life Member submit the Life Membership Form) 

_ Life Membership – for all New Life Members

National with National Life Member Dues and Regional with Regional Life Member Dues

_ Teen Roster 

3. Bank Transmittal Forms

National Operation Account Bank Transmittal Per Capita Tax and Bonding form must be submitted with the appropriate dues for the 2007-2008 Fiscal Year. Send a copy along with the following:

_ Chapter check to the National Treasurer at Headquarters, 

_ A copy of chapter check to the National Recording Secretary,

_ With chapter check to me your Central Region Secretary/Treasurer. 

NOTE: October 31, 2007 is the deadline for submitting the requested material. Please be reminded that $100 MUST accompany any report (partial or complete) received after the October 31, 2007 deadline.
